If you would like to be an exhibitor at our 35TH Annual Shoulder Course, please fill out the Exhibitor Registration form and Exhibit badge form and fax it to 951-695-6801 or email it to sdsi_info@shoulder.com. Booth/Table Top assignments are made on a first come/first served basis.

We have 48 Booths and 8 Table Tops available.

Booths:  Booth sizes are 10’ wide x 10’ deep booth. Booth prices are $3,500. Booth pricing includes 8’ high blue back drape, 3’ high blue side dividers, and a 7” x 44” identification sign.

Table Tops: Table top exhibits are 6’ tables. $2,500 per table. Price includes one 6’ table, white table drape, and one chair.

Registration fees include three company representatives per booth/table top.

Additional representatives may register $250 per person. Please submit names for all badges using the badge request form no later than May 31, 2018.

ADDITIONAL BADGES ORDERED AT THE COURSE WILL BE $350 per person.

Full payment is required for registration. $100 administrative fee will be retained upon cancellation. No refunds will be made after May 1, 2018.
